I, Chris Laux, do solemnly offer these my responses to The Road to Lisp Survey:

When did you first try Lisp seriously, and which Lisp family member was it?

Common Lisp in 1999

What led you to try Lisp?

I came across a copy of "Ansi Common Lisp" by Paul Graham, the German translation, for 3.33 DM (Deutschmark) - about $2-3 in those days. For that price of course, I couldn't resist!

What other languages have you been using most?

BASIC (1991), Pascal, C/C++, Assembly, Java in roughly that order.

How far have you gotten in your study of Lisp?

I guess I'm fluent, but far from mastery.

What do you think of Lisp so far?

I dislike using other languages, although I sometimes have to for work. Lisp is great, but I have objections to the whole idea of using a textual programming language.
